Why Should You Apply for Grant?

Doc's Dental Grant provides you with the opportunity to better afford implant dentistry. It is free to apply for and is available to anyone that can be safely treated at our facility. To be eligible for consideration of a Grant, a consultation must be performed.

• Consultation review of Medical History and current medications, X-Rays and examination must be done to make a "Treatment Plan".

• Consultations usually take about 30-60 minutes during which you can discuss your main concerns with the dentist to develop a treatment plan. The Treatment Plan must be done so we know what you need and how much you may qualify for.

Note: The dentist will not compromise the function or health of your mouth to accommodate an implant procedure. Accordingly, the dentist will advise in advance if you require any basic dentistry such as root canals, fillings, cleanings or extractions with your regular dentist, prior to implant treatment.

How the Process Works

• An applicant submits a request for a Grant at our website.

• The applicant’s information is confirmed by our staff.

• Staff will Book the consultation visit for examination, x-rays and treatment plan.

• We determine eligibility based on criteria of donors and available funds.

Qualifying As a Candidate

To determine your eligibility, once you submit your application, we will confirm your information and a consultation appointment will be scheduled. Following completion of consultation visit including:

• Your dental history.

• Your oral health assessment

• What kind of dental work you want done.

• How soon you wish to start treatment.

• Presuming you are a good candidate for implant dentistry, we will review the costs and help with financing if needed.

Awarding of Doc's Grants

Full Grants

Full Grants are awarded through Donors who are anonymous unless they reveal themselves directly to the recipients.

The donors are usually a family member or someone who knows and refers a patient who needs implant dentistry and we respect their wishes to remain anonymous.

Partial Grants

Partial Grants are awarded to any qualified applicant on a per capita, percentile basis, distributed proportionally based on the number of applicants in the distribution pool, the value of their recommended treatment plan, and the amount of proceeds available for distribution.

Once becoming eligible for a partial grant, applicant's must proceed with their implant dentistry work within 28 days or the grant will be awarded to the next qualified applicant.  Each applicant must submit a signed affidavit in order to confirm acceptance.

Applications Processing and Consultation Appointment are dependent on the number of applications and available grants.

Treatment must be started within 28 days of award of a Grant. You may only apply one time per year.

DENTAL GRANTS

GRANT APPLICANT/RECIPIENT AGREEMENT

Effective January 1, 2023

As an applicant, or potential recipient of a grant, I agree to the following terms:

1.) I am responsible for informing of any change or changes in my name or address during the period of time comprised of applying for a Grant, and receipt of any Grant proceeds that may be awarded.

2.) If I am awarded a Grant, I understand I have 28 days to accept or the Grant will be awarded to another Applicant.

3.) I certify that I am at least 18 years of age.

4.) By submitting this application, I am giving permission to Doc's Dental Implants & Crowns to contact me via email, via text, via a phone call or via regular mail at the email, the phone numbers and/or addresses provided in this application.

5.) Any grants awarded are towards the actual dental implant treatment ONLY. Applicants are responsible for arranging for transportation and/or lodging at their own cost. A dental treatment may require several visits, so applicants should factor in any travel and/or lodging costs associated with their treatment and understand that these costs are the applicant's responsibility. Submitting an application for a Doc's Grants confirms you have read, understand and agree to the terms of these guidelines and agree to comply with them.
